PRIMARY PURPOSE Provides pharmacy services to patients, nurses, physicians, and other medical staff by interpreting and evaluating the validity and safety of orders and prescriptions. Acts as a therapeutic consultant in their practice area on core and more advanced medication issues. May perform duties in a lead pharmacist role for a shift. Performs education and training of students and residents.

MNIMUM SPECIFICATIONS

Education

  • Must have a bachelor's degree in pharmacy or a Pharm D. degree from an accredited school of pharmacy.

Experience

  • - Must have two (2) years' experience.

Equivalent Education and/or Experience

  • - Completion of a PGY1 residency program and one year pharmacist experience may be considered equivalent to three years of required pharmacist experience.

Certification/Registration/Licensure

  • - Must be a Registered Pharmacist in the State of Texas.
  • - Prefer preceptor certificate from the State Board of Pharmacy to be eligible to accept students on rotations.

Skills or Special Abilities

  • - Must possess good oral and written communication skills.
  • - Must have basic computer skills.
  • - Must possess knowledge of drugs, procedure, equipment, common disease states and patient care monitoring within the assigned care areas.
  • - Required appropriate pharmacy specific mathematical skills.
  • - Must possess good organizational skills.
  • - Additional Requirements:
  • Must have delivered one major, structures presentation to a physician group and/or healthcare providers group(s) or three minor in-services or presentations.

Responsibilities

  • 1. Demonstrates effectiveness and efficiency in routine and complex order preparation/review/verification by authorized prescribers for therapeutic appropriateness, duplications, side effects, effectiveness, lab monitoring, etc. Facilitates and ensures appropriate formulary use. Reviews and interprets literature and applies insights to complex medication orders/drug information queries related to medication orders and applies outcomes to general and specific patient situations/populations. Communicates/shares findings with other pharmacists.
  • 2. Demonstrates accuracy and efficiency in computer entry/verification and/or preparations of medication orders/order processing/order preparations including specialty orders. 3. Maintains a clinical specialty or focus area and deals with complex patients and populations. Works closely with prescribers and nursing staff to optimize outcomes related to their medication therapy using integration of patient variables, drug knowledge and professional judgment. Monitors literature to maintain expertise and apply practical clinical applications into daily practice.
  • 4. Performs QM, QA and PI on a monthly basis as part of the nursing/pharmacy liaison responsibilities. Identifies and implements necessary changes and monitors the results of these programs to ensure continues performance improvement and area compliance. Provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards to designated areas.
  • 5. Educates and communicates effectively with patients on medication therapies to ensure that patients are informed of proper drug usage and knowledge of potential side effects. Consults and evaluates all available patient information both documented and through patient interview to individualize medication regimens and adherence tools communicated to the patient and caregiver.
  • 6. Acts as a resource to physicians, nurses, and patients concerning questions/support related to drug distribution and drug information services in line with the hospital and departmental procedures. Consults scientific literature and applies practical clinical implications to specific patient¿s cases. Actively attends and participates in departmental staff meeting and staff development programs to maintain current knowledge and understanding or procedures and/or new programs.
  • 7. Provides preceptorship and mentoring, training of pharmacy students, pharmacy residents, technicians on individual teams and/or in areas as well as new employees and other healthcare providers. Activities should include providing both practical and didactic experiences. Acts as a role model for future pharmacists and/or as a positive representative of clinical pharmacist functions and abilities.
  • 8. Attends, participates and provides feedback in journal club, in-services, resident topic discussions, and resident/student case presentations.
  • 9. Documents interventions for resolved prescribing, formulary and therapeutic issues. Identifies and discusses adverse drug events with the care team and documents in Patient Safety Net. Suggests and implements with pharmacy leadership new processes, systems or procedures to enhance patient safety and the prevention of adverse events or errors in any step of the medication management process.
  • 10. Effective in day-to-day supervision of all subordinates (or peers if performing in a lead rph shift role). Provides guidance and direction to and on-site supervision of all technicians assigned to same shift/area. Ensures quality and accuracy of all activities by technicians. Assists with the selection, mentoring, and evaluation of support personnel to ensure maximum utilization of individuals while providing optimal services. Effective in professional interactions with peers and subordinates. Contributes to overall maintenance of quality and compliant pharmacy services.
  • 11. Identifies ways to improve work processes and improve customer (internal as well as external) satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals and mission of Parkland.
  • 12. Stays abreast of the latest developments, advancements, and trends in the field of Clinical Pharmacy and applicable laws, regulations, policies and guidelines that impact Pharmacy. Participates and attends local, regional and national seminars/workshops, reading professional journals, participates in professional organizations, and maintaining certification or licensure. Integrates knowledge gained into current work practices. Promotes adherence to all laws and procedures in daily practice and mentoring of peers. Seeks guidance from management as necessary to gain understanding or suggest improvements.
  • 13. Demonstrates an active interest in promoting the profession of pharmacy by participating in in-service education and didactic training, present to local, state, or national meetings, participating in department or area committees, participating in community service/outreach activities, assisting in MUE or performance improvement projects, participating in recruitment activities and/or serve in a leadership role in a professional organization.
  • 14. The pharmacist will complete training on the current cash collection system as well as stay abreast of current cash handling policies and procedures to maintain compliance and support to the pharmacy technician.
