Primary Purpose


The Senior Patient Safety /Risk Manager lead and facilitate all levels of activity associated with the patient safety and clinical risk management program. Oversees health care risk management and patient safety foundational elements such as safety post review and investigation. In depth investigations of cases that may require root cause or apparent cause analyses as well as facilitation of root cause and apparent cause analyses and Failure Mode Effects Analysis (FMEA). This position leads projects identified through safety posts, common causes, industry trends or other means. The Senior Patient Safety /Risk Manager assists peers through the orientation process, leads disclosure just time training with colleagues, and develops strong relationships with key leaders throughout the network.  

Minimum Specifications


Education:

  • Bachelor’s Degree in Nursing, or a clinically related field of study from an accredited college or university.

Preferred Education:

  • Master’s Degree in Risk Management, Patient Safety, Healthcare Administration or a related field of study from an accredited college or university.

 

Experience:

  • 6 plus years of risk management and/or patient safety experience in an acute care facility.
  • 6 plus years of clinical experience in an acute care setting.

 

Equivalent Education and/or Experience

  • N/A

 

Certification/Registration/Licensure:

  • Certification through a National Accreditation Board in Health Care Risk Management (CPHRM) or Patient Safety (CPPS) through the American Society for Healthcare Risk Management (ASHRM) or Institute of Healthcare Improvement (IHI) for the CPPS is required.
  • Current licensure is required in applicable field of study. Licensure must be maintained throughout employment.
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-700.A: National Incident Management System (NIMS) An Introduction or obtain within 30 days of placement in role. Incumbents as of 7/5/2017 must obtain by September 3, 2017 (90 days from addition of certification.)
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-100.HCB: Introduction to the Incident Command System (ICS 100) for Healthcare/Hospitals or obtain within 30 days of placement in role. Incumbents as of 7/5/2017 must obtain by September 3, 2017 (90 days from addition of certification.)
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-200.HCA: Applying ICS to Healthcare Organizations (ICS 200) or obtain within 30 days of placement in role. Incumbents as of 7/5/2017 must obtain by September 3, 2017 (30 days from addition of certification.)

Responsibilities:

  1. Receives, manages and investigates all assigned safety posts in Safety Center. Assesses for accuracy, severity, and trends. Works with local leaders of assigned areas as needed. Escalates cases of concern to departmental leadership in timely manner.
  2. Facilitates all activities associated with all levels of investigation for all cases undergoing a RCA/ACA reviews. Performs other forms of investigation such as Failure Mode Effects Analysis (FMEA) s, assault allegations, and all other forms of safety and risk investigation.
  3. Identifies deviations in clinical standards of care and patient rights violations which may have patient safety implications and/or risk exposure to the hospital. Identifies and escalates significant events through appropriate channels across the health system.
  4. Conducts Sentinel event, NQF, TDSHS and CMS investigations in accordance with departmental SOP’s.
  5. Enters all investigations and reviews in the event tacking log so that department productivity and metrics can be analyzed and trended.
  6. Develops deep collegial relationships with assigned areas. Provides data and educational information at routine intervals. Rounds weekly on assigned areas to field questions from front line staff and validates transmission of shared learnings and previous information downloads.
  7. Demonstrates command of the foundational elements of a comprehensive enterprise risk management (ERM) program and evaluates risks from the PHHS global perspective.
  8. Leads and/or participates in patient safety initiatives for assigned areas and within the division which may minimize patient harm and/or improve patient outcomes. Assists and contributes to patient safety program development along with peer group, divisional colleagues and immediate Patient Safety Director. Collaborates with others as needed to ensure compliance with project deadlines and/or regulatory requirements.
  9. Communicates professionally and effectively with all levels throughout the health system to ensure optimal uptake of assignments and sharing of information.
  10. Serves as a trainer, facilitator, and day-to-day liaison to the organization. Provides education and/or subject matter expertise as needed or requested. Attends various meetings, committees, in-services and/or orientations as needed or requested.
  11. Leads projects such as FMEAs or risk assessments for identified vulnerabilities or upon request.
  12. Maintains comprehensive and appropriate documentation of all projects, investigations and action plans, in accordance with Parkland and industry standards.
  13. Completes other special projects as assigned.
  14. Identifies and analyzes the design of jobs, work processes, work flows, etc. for the area and implements appropriate changes to improve effectiveness, productivity, and efficiency that support the overall goals of the department and Parkland.
  15. Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  16. Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
  17. Develops and monitors annual budgets that ensure department has the necessary funds to carry out the goals and objectives that have been established for the department.
  18. Develops, implements, monitors, and revises annual goals and objectives for the department that support the missions and objectives of Parkland.
  19. Selects, trains, schedules, motivates, supervises, and evaluates employees making recommendations for disciplinary actions up to and including termination, to ensure maximum utilization of individual and group capabilities. Ensures that assigned employees receive opportunities to further their knowledge.
