PRIMARY PURPOSE

Responsible for education coordination within the Care Management Department and responsible for assisting with grant development and grant implementation within Care Management.

MINIMUM SPECIFICATIONS

Education:
- Must have a Master's degree in Social Work from an accredited school of social work or a degree in Nursing from an accredited school of nursing.

Experience:
- Must have five years of hospital based experience to include one year of experience of grant development and staff education.

Equivalent Education and/or Experience
- May have an equivalent combination of education and/or experience in lieu of specific education and/or experience as stated above.

Certification/Registration/Licensure:
- Must be currently licensed as a Licensed Master Social Worker by the Texas State board of Social Work Examiners.
- Or, must be currently licensed as an RN by the Texas Board of Nurse Examiners or have a valid temporary license with the Texas Board of Nursing or valid Compact RN license.

Skills or Special Abilities:
- Must have excellent oral, written and organizational skills.
- Must have excellent interpersonal skills and be able to communicate and collaborate effectively with a diverse group of patients, families and healthcare staff.
- Must have grant-development skills.
- Must be detail oriented, self-directed, and capable of priority-setting and problem solving.
- Must be able to demonstrate a working knowledge of PC operations and the ability to use work processing software in a Windows environment.
- Must be able to demonstrate patient centered/patient valued behaviors.
