Primary Purpose

Responsible for developing and coordinating outreach and education activities in pertinent areas of healthcare and providing support in the areas of recruitment and supervision of volunteer and staff personnel. Responsible for conducting education in classroom setting as well as one on one, and promoting positive health practices among the targeted population.

 

Education

•Must have a Bachelor Degree in a healthcare field, social services field, business administration, or related field.

 

Experience

•Must have three years of experience in community health or comparable organization.

Equivalent Education and/or Experience

•May have an equivalent combination of education and/or experience in lieu of specific education and/or experience as stated above.

 

Skills or Special Abilities

•Must have excellent oral and written communication skills.

•Must be organized, detail oriented and have demonstrated management ability.

•Must demonstrate knowledge of community resources and services available for the targeted population.

•Must be able to demonstrate client centered/client valued behavior.

•Must have excellent interpersonal skills, particularly in working with a diverse population.

 

Responsibilities

1.Responsible for all aspects of the Dallas Healthy Start (DHS) Health Education module in culturally sensitive environments in community, clinic settings and virtual settings. Schedules and implements health education initiatives for the DHS target population, including perinatal and car seat classes in target zip codes. Works closely with clients as an integral participant in client education and development.

2.Coordinates training and education for case management staff as needed and works with WISH Departments and Parkland departments on coordination and integration of education focused in various aspects of maternal and child health.

3.Responsible for assessing the education needs of the Maternal Child Health (MCH) client, developing, implementing and evaluating materials and actively participating in community coalitions, to reduce high risk behaviors and promote healthy choices within the DHS target population.

4.Serves as an advocate for client/patient services development, as well as a community resource person, and provides consultation to community, client and organizations to maximize the understanding and development of health education programs.

5.Coordinates health education activities with community action network and case management and participates in outreach activities to educate the community, maintain and, establish community partnerships.

6.Assists in identifying new clients and refers them for case management.

7.Assists in the development of annual goals, as well as the implementation and monitoring of annual goals for the DHS program that support the mission and objectives of Parkland and the WISH division.

8.Serves on various committees as assigned to represent DHS & WISH.

 

Job Accountabilities

  • Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
  • Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
