PRIMARY PURPOSE Assesses, plans, implements and coordinates care for the minority HIV infected patient population, to assist, serve and identify their needs for referrals and planning to ensure retention in care. MINIMUM SPECIFICATIONS Education: - Must have a High School diploma. Experience: - Prefer two (2) years of experience working with HIV positive and minority populations. Equivalent Education and/or Experience: - May have an equivalent combination of education and experience to substitute for both the education and the experience requirements. Certification/Registration/Licensure: - None. Skills or Special Abilities: - Must be able to demonstrate oral and written communication skills. - Must be skilled in communicating with and facilitating dialogue between health care professionals, individuals and their families. - Must be able to demonstrate active listening, empathy, relationship building, and interpersonal skills. - Ability and willingness to disclose HIV lived experience with peers, professionals and the public. - Must demonstrate knowledge and ability to integrate health and the PHHS health system, into the culture and language of the community. - Must have knowledge of communities they serve. - Ability to actively participate as a team member of a health-care team.
