PRIMARY PURPOSE: Patient Navigator provides individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality medical and psychosocial care. The Navigator will collaborate with internal and external teams to provide access to exceptional care delivery. Facilitates clinic-based services as well as any available community resources to support a patient's socioeconomic needs and quality of life.

 

MINIMUM SPECIFICATIONS

Education:

- Must have high school diploma or GED.

 

Experience

- Minimum of six (6) years of health care in an office/clinic/hospital setting or human services experience.

 

Certification/Registration/Licensure

- Must obtain Oncology Patient Navigator, Certified Generalist, (OPN-CG) within the first 12 months of employment.

 

Skills or Special Abilities

- Must be able to demonstrate time management, organizational, and interpersonal skills with emphasis on attention to detail including oral, written, communication, and advanced computer skills.

- Must be able to support care that is culturally sensitive and linguistically appropriate.

- Must be able to utilize motivational interviewing skills to stress the importance of adherence to care/treatment plans.

- Must be able to demonstrate the ability to instruct and/or lead patient education.

- Must be able to assist with patient care coordination to ensure the continuation of care.

- Must have EPIC EMR system experience.

 

Responsibilities

1. Communicates with patients, caregivers, and care teams to ensure patient adherence to care plans. Serves as a patient advocate, focusing on patients¿ needs, rights, confidentiality, and cultural preferences.

2. Promotes collaborative practice among patients, their families, and multidisciplinary healthcare team members. Serves as a resource person for specific clinical and patient care issues, helping to negotiate desirable patient outcomes.

3. Conducts patient assessment and refers to appropriate resources to facilitate access to care (i.e., transportation, childcare, and financial counselors).

4. Provides education in various settings to increase patient/community knowledge of Parkland services, referral process, authorization process, plan coverage, and community resources.

5. Refers patients to primary care/referring/specialty providers as needed to ensure quality and continuity of patient care.

6. Maintains proper documentation in EPIC and other relevant software.

7. Attends meetings and acts as a resource for the care team and other staff.

8. Maintains a positive relationship with all Parkland internal and external partners.

9. Identifies and implements process improvement opportunities.

10. Supports required reporting for accreditations, grants, and other entities.

11. Develops training and monitors annual goals and objectives set forth by department/program administration.

12. Maintains knowledge of applicable rules, regulations, policies, laws, and guidelines that impacts the relevant department areas. Develops effective internal controls that promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper understanding.

13. Stays abreast of the latest developments, advancements, and trends in the relevant fields by attending seminars and meetings and reading professional journals.

14. Performs other duties as assigned.
