PRIMARY PURPOSE Responsible for working with key stakeholders to manage a designated program which may include development and implementation, staffing, clinical support, budget, acquiring and maintenance of accreditations, and coordination of community agency services and educational activities depending on assigned area.

 

 MINIMUM SPECIFICATIONS

Education - Master's degree in public health, Health Administration, Business Administration, or health-related field required.

Experience - Must have five (5) years of related experience and/or four (4) years of management/supervisory experience.

- Preferred three years working within a population health community setting.

- Prefer experience working in an urban healthcare system.

- Prefer experience with State and/or Federal grants administration and program/project management.

 

Equivalent Education and/or Experience - May have an equivalent combination of education and/or experience in lieu of specific education and/or experience as stated above.

 

Certification/Registration/Licensure

- For Diabetes Program Manager: - RN Only: - Must have a current, valid RN license or temporary RN license from the Texas Board of Nursing, or valid Compact RN license.

- RD Only: - Must be a Registered Dietitian by the Commission on Dietetic Registration and a Licensed Dietitian by the Texas Department of Licensing and Regulation.

 

- Must have current healthcare provider / Heartsaver CPR course completion card from one of the following within 90 days from hire date and/or job placement:

  • - American Heart Association
  • - American Red Cross Rescuer
  • - Military Training Network

 

- Must have a Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) from the Certification Board for Diabetes Care and Education (CBDCE) (previously called National Certification Board for Diabetes Educators (NCBDE).

- Specialty certifications related to the field, program, and disease site preferred (project management or clinical certification).

Responsibilities

1. Responsible for program evaluation, required performance standards, quarterly data reports, annual reports, and preparation of annual program requests.

2. Creates data and reporting infrastructure to inform programmatic changes to improve utilization of patient care services.

3. Conducts an ongoing evaluation of all activities to determine their impacts and outcomes and ensure continuous quality improvement of services.

4. Supports patient education, patient experience, and care coordination projects that would allow optimal and streamlined clinical care across the care continuum.

5. Supports the management of patient advisory groups and patient support groups.

6. Responsible for daily program management and supervision of program and support staff. Selects, trains, motivates, supervises, and evaluates assigned staff, if applicable, to ensure maximum utilization of individual and group capabilities, while providing optimal service to both internal and external customers. Ensures assigned employees receive opportunities to further their knowledge.

7. Supports development and monitoring of annual budgets that ensure the programs have the necessary funds to carry out the goals and objectives that have been established for the department.

8. Maintains knowledge of applicable rules, regulations, policies, laws, and guidelines that impact programs.

9. Develops effective internal controls that promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper understanding.

10. Responsible for overseeing the development and maintenance of a programmatic dashboard, for the purpose of internal and external reporting with regard to performance and evaluation criteria. Assists in the development, implementation, and monitoring of program goals. Monitors and reports activities supporting departmental goals according to established standards. Evaluate and revise work-related.
