Primary Purpose

Responsible for providing comprehensive specialty care services for people living with diabetes in accordance with established protocols with a multi-disciplinary plan of care to bridge the gap and integrate clinical and self -management aspects of care.

 

Minimum Specifications 

Education: Must be a graduate from an accredited school of nursing.

Experience: 2+ years of clinical experience in the direct care of people with diabetes and associated conditions. Diabetes self-management education and support experience preferred.

 

Certification/Registration/Licensure 

  • Must have current, valid RN license or temporary RN license from the Texas Board of Nursing, or valid Compact RN license.
  • Must have or obtain Certified Diabetes care and Education Specialist (CDCES) or certified Diabetes Educator (CDE) or obtain within 2 year of hire date. 

 

Skills or Special Abilities 

  • Has a working knowledge of diabetes technology.
  • Excellent verbal and written communication skills including the ability to communicate effectively and professionally across disciplines. Ability to communicate complex information in understandable terms. 
  • Strong interpersonal skills with the ability to establish and maintain effective working relationships across and beyond the organization. 
  • Excellent analytical and problem-solving skills. 
  • Strong time management skills with the ability to handle multiple demands and respond to rapidly changing priorities. 
  • Ability to write clearly and succinctly as well as a high level of attention to detail. 
  • Proficient computer and Microsoft Office skills. 
  • Knowledge of Texas Medicaid, National Committee for Quality Assurance (NCQA), the Uniformed Managed Care Contract, and the Uniform Managed Care Manual. 

 

Responsibilities:

Member Outreach

  • Conducts member outreach to ascertain the members willingness to modify current practices and behaviors executing appropriate disease management interventions.
  • Completes comprehensive assessments for each member including emotional and behavioral health, interpret personal health data, develop an individualized care plan based on the members assessed needs and goals that promote successful self-management.
  • Documents all individual contacts /visits and outcomes data in the DM documentation system according to guidelines in a timely manner.

 

Care Coordination 

  • Performs clinical task within the scope of practice defined by the licensing board.
  • Collaborate, advocate, and confer with other team members in developing person-centered diabetes plan.
  • Participates in interdisciplinary team meetings to support members in the accomplishment of their goals.
  • Demonstrates appropriate critical thinking and analytical skills in clinical practice. Integrate current evidence-based practices with clinical knowledge and member / family preferences for the delivery of optimal care.
  • Acts as a liaison and member advocate between the member, family, physician and facilities.
  • Coordinates the member¿s access to multiple service systems which may include obtaining a medical home, instructing on appropriate use of the medical home, assisting member with overcoming transportation barriers, informing member of available community resources and how to request or use these resources.
  • Assists member with navigating the healthcare system and community-based resources.
  • Participates in process improve initiatives related to disease management program outcomes.
  • Ensures disease management reporting requirements are met. 

 

Education 

  • Follows the Diabetes Education Accreditation Program requirements of diabetes self-management and supports guidelines.
  • Encourages and supports members with achieving an optimal level of wellness by understanding triggers and adopting habits conducive to improved management of their chronic condition.
  • Educates and coaches members on how to improve the management of their chronic condition using a culturally appropriate format.
  • Provides the appropriate level of support to improve the members adherence to the medical treatment plan.

  • Ensures work is carried out in compliance with regulatory and/or accreditation standards as well as contractual requirements.
