PRIMARY PURPOSE
Responsible for influencing and supporting long-lasting behavior changes in members with chronic diseases such as asthma and diabetes. Serve as an advocate empowering members to take control of their health through disease education and continuous support.
MINIMUM SPECIFICATIONS
Education
- Graduate of an accredited American Medical Association (AMA) Respiratory Care Program required.
- Associate or bachelor's degree in respiratory care preferred.
Experience
Three (3) years of experience working as a Respiratory Therapist.
Two (2) years of asthma education experience preferred.
Certification/Registration/Licensure
Current licensure in good standing as a Respiratory Care Practitioner in the State of Texas and Registered by the NBRC.
BCLS certification current upon hire.
Valid Driver's License.
Asthma Education certificate (AE-C) required within two (2) years from date of entry into position is preferred.
Skills or Special Abilities
Demonstrated ability to communicate effectively and professionally both verbally and in writing.
Ability to establish and maintain effective working relationships across and beyond the organization.
Excellent organizational and time management skills with the ability to handle multiple demands and respond to rapidly changing priorities. Knowledge of Federal and Texas Medicaid regulations.
Ability to work independently and with minimal supervision.
Strong member advocacy skills.
Proficient computer, disease management specific software, and Microsoft Office skills.
Responsibilities
Member Outreach Conduct member outreach activities to ascertain the members willingness to modify current practices and behaviors to avoid long term complications associated with their chronic condition. Execute appropriate disease management activities and interventions ensuring all of the member¿s needs are met. Promote member engagement and ongoing participation in the Disease Management Program for asthma. Establish trusting relationships to support members and their families with understanding and implementing asthma care plans to improve or restore an optimum respiratory health status for the member. Assist and/or participate in outreach and advocacy opportunities as requested to increase awareness of chronic conditions and to engage members in care coordination services. Serve as a member advocate ensuring the member's values, preferences, and expressed needs are met as part of the care plan. Demonstrate cultural sensitivity and competency for assigned members. - Care Coordination Perform clinical tasks within the scope of practice defined by the licensing board. Demonstrate appropriate critical thinking and analytical skills in clinical practice. Integrate current evidence-based best practices with clinical knowledge and member/family preferences for the delivery of optimal care. Conduct in-person visits with members to (1) complete an assessment of the member¿s respiratory status and needs and (2) support compliance with care plan goals and outcomes in collaboration with the interdisciplinary care team. Participate in multidisciplinary team rounds identifying goals to assist members with accomplishing their treatment plan. Working collaboratively with the member and the member's primary care givers, review health conditions and concerns to develop an individualized plan of care to address respiratory issues. Act as a liaison and member advocate between the member, family, physician and facilities or agencies. Coordinate the member's access to multiple service systems which may include obtaining a medical home, instructing on appropriate use of the medical home, assisting member with overcoming transportation barriers, informing member of available community resources and how to request or use these resources. Escalate issues or matters of concern to the appropriate level for assistance in a timely matter. Maintain current and accurate documentation ensuring confidentiality and compliance with HIPAA regulations is maintained. Participate in process improvement initiatives related to asthma program outcomes. Ensure disease management reporting requirements are met. Demonstrate compliance with NCQA Disease Management and Population Health Standards. - Education Provide family centered disease-specific education and care coordination for members. Utilize in-home visits to provide direct education on asthma and to address any environmental issues. Provide recommendations to mitigate these issues in accordance with NHLBI guidelines. Educate members and the member¿s caregivers on respiratory conditions, proper medication use and provide support on correct inhaler usage as needed. Encourage and support members with achieving an optimal level of wellness by understanding triggers and adopting habits conducive to improved management of their chronic condition. Educate and coach members on how to improve the management of their chronic condition using a culturally appropriate format. Instruct members on how to access program or community resources arranging follow-up such as mailing of educational materials, contacting community resources, and/or facilitating physician visits. Provide an appropriate level of support to improve the member's adherence to the medical treatment plan. - Regulatory Ensures work is carried out in compliance with regulatory and/or accreditation standards as well as contractual requirements. - Professional Accountability Promotes and supports a culturally welcoming and inclusive work environment. Demonstrates accountability and responsibility by independently completing work, including projects and assignments on time, and providing timely responses to requests for information. Maintains confidentiality at all times. Performs other work as requested that is reasonably related to the employee's position, qualifications, and competencies.
