PRIMARY PURPOSE
Assists patients in their transition from the hospital to outpatient setting. Responsible for the inpatient evaluation and coordination of the plan of care with all members of the multidisciplinary health care team. Monitors and assists the patient and family in the outpatient setting through phone calls and home visits. Monitors patient's level of compliance with the plan of care to avoid readmissions and Emergency Department visits to maximize patient well being and self-reliance.

MINIMUM REQUIREMENTS
Education:
- Must be a graduate of an accredited school of Nursing or have a Master's degree in Social Work.

Experience:
- Must have four years of hospital or community based patient care nursing or social work experience, preferably in assigned clinical area.
- Prefer case management, utilization review, home health care and/or discharge planning experience.

Equivalent Experience
- May have an equivalent combination of education and experience to substitute for the experience requirement.

Certification/Registration/Licensure:
- Must have current healthcare provider CPR course completion card from one of the following:
o American Heart Association
o American Red Cross Rescuer
o Military Training Network.
- Must have a valid Texas Driver License (Class C)
- Prefer certification from the Commission for Case Management Certification.
- Prefer Texas Department of Health Targeted Case Management (TCM) 90- day training certificate.
- Prefer American Nurses Credentialing Center (ANCC) Nursing Case Management Certification.
- RNs Only: Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license.
- Social Workers Only: Must be licensed by the Texas State Board of Social Work Examiners.

Required Tests for Placement:
- RNs Only: Applicants must pass (85% or above) Dosage Calculation Exam prior to hospital orientation. Current Parkland employees requesting reassignment to role must have current Dosage Calculation Exam on file.

Skills or Special Abilities:
- Must be able to be insurable and have access to a personal vehicle.
- Must be able to communicate and collaborate effectively with a diverse group of patients, families and healthcare staff.
- Must be able to demonstrate a working knowledge of specific patient populations, and be able to demonstrate knowledge of disease processes affecting this group.
- Must be able to demonstrate a working knowledge of PC operations and the ability to use word processing software in a Windows environment.
- Must be able to demonstrate a working knowledge of the laws and regulations governing Medicare, Medicaid and community-based funding sources.
- Must be self-directed and capable of priority setting and problem solving.
- Must be able to demonstrate patient centered/patient valued behaviors.
- Fluent in Spanish strongly encouraged.
- RNs Only: Provides care to assigned patient population in accordance with the current State of Texas Nursing Practice Act, established protocols, multidisciplinary plan of care, and clinical area specific standards.
